Welcome to our practice!!!
We are happy you have selected our office for your care. We are dedicated to providing high quality medical care and service. We hope the following information assists you as you prepare for your visit.
Practice hours: Monday thru Friday 830 am to 5 pm. We close the office for one hour for lunch.
Appointments: Dr. Hess usually sees patients Monday, Wednesday and Friday afternoons and Tuesday and Thursday mornings. Dr. Sandeen usually sees patients Monday, Wednesday and Friday mornings and Tuesday and Thursday afternoons. We respect your time and do our best to see our patients as close to their appointment time as possible. On occasion we are even able to see patients ahead of their scheduled appointment time. If we are running behind, we will try to keep you informed and we appreciate your understanding.
New patient appointments: For your first visit to our office, please arrive approximately 20 minutes prior to your appointment as it takes time to complete the registration process. If you will be utilizing insurance for your visit, please remember to bring that information with you as well as a photo ID. If you have not already done so, please log into our secure patient portal (Patient Log-In) and complete the pre-registration, HIPPA, financial and health history forms.
Accepted forms of payment: We accept Visa, Mastercard, American Express, cash and checks.
Insurance: Our physicians are participating providers for United Healthcare and their associated plans, Blue Cross Blue Shield, Healthnet of Arizona, Pacificare, Secure Horizons and Medicare. Dr Hess is also contracted with GreatWest.
We understand that medical practices require a lot of paperwork and information. In an effort to make this more convenient for you, we are providing a link to our secure patient portal. Here you will have the opportunity to pre-register, complete your health history forms and ask questions in a secure environment.
Our goal is to try to make things as easy for you as possible. If you prefer not to log into our secure patient portal and create an account, you may click the following link to down load our new patient paperwork:
Prescription refills: Our physicians prefer that you request your medication refills at the time of your office visit. Should you otherwise wish to request medication management, please log into your account in our patient portal or contact our office during regular business hours (Monday - Friday, 830 am to 5 pm).
