Welcome to our practice!!!
We are happy you have selected our office for your care. We are dedicated to providing high quality medical care and service. We hope the following information assists you as you prepare for your visit.
Practice hours: Monday thru Friday 830 am to 5 pm. We close the office for one hour for lunch.
Appointments: Dr. Hess usually sees patients Monday, Wednesday and Friday afternoons and Tuesday and Thursday mornings. Dr. Sandeen usually sees patients Monday, Wednesday and Friday mornings and Tuesday and Thursday afternoons. We respect your time and do our best to see our patients as close to their appointment time as possible. On occasion we are even able to see patients ahead of their scheduled appointment time. If we are running behind, we will try to keep you informed and we appreciate your understanding.
New patient appointments: If this is your first visit to our office, please arrive approximately 20 minutes prior to your appointment as it takes time to complete the registration process. If you will be utilizing insurance for your visit, please remember to bring that information with you.
Insurance: Our physicians are participating providers for United Healthcare and their associated plans, Blue Cross Blue Shield, Healthnet of Arizona, Pacificare, Secure Horizons and Medicare. Dr Hess is also contracted with GreatWest.
We understand that medical practices require a lot of paperwork and information. In an effort to make this more convenient for you, we are providing a link to our forms so you may print and complete them in the privacy of your own home or office. We have several forms and all must be completed. Thank you for your help and understanding.

Prescription medication: If you have questions about your prescription or would like to request a refill, please contact us during office hours (Monday thru Friday 830 am to 5 pm) at 520/297-3300.
